MENTORSHIP DD/SLS WAIVERS

Our primary focus is to assist in becoming independent with moderate level support in social and community settings according to their individual service plan and abilities.

MASSAGE THERAPY DD/SLS/CES WAIVERS

Our licensed professional therapists can relieve some minor pain, help injuries, improve circulation, relieve stress, increase relaxation, and aid in the general wellness of each individual. Services are in home, or an office setting.

RESPITE HOME PROVIDER (RHP) DD/SLS/CES WAIVERS

Short to Long term, and/or emergency (if applicable case by case) non-medical care to allow caregivers an interval of rest, relief, or time for his/her own needs. Our Responsibilities: Support the needs of the individual in which respite will be offered, to also provide the best safety and secure atmosphere to each individual under our care. Each individuals service plan will vary according to their ability. RHP does not consist of a hands on medical service. *Parents and or guardians may qualify to utilize Family for Respite their services.

SUPPORTED COMMUNITY CONNECTOR DD/SLS

To utilize the community as a learning environment to provide individuals access to, as well as participation in, educational activities and events in social settings throughout the community. We will explore services appropriate to each individual based on their ability and service plan. Monitor and supervise closely to enhance socialization and community access capabilities.The Community Services we will look at will be those that enhance each individuals way of perceiving and understanding life from their perspective *Parents and or guardians may qualify to utilize Family SCC for their services.

COMMUNITY CONNECTOR CES

We will explore community services appropriate to each child based on their ability and service plan. Closely monitor and supervise to ensure a safe and secure environment according to their socialization capabilities.The Community Services we will look at will be those that enhance each individuals way of perceiving and understanding life from their perspective *Parents and or guardians may qualify to utilize Family SCC for their services.

BASIC/ENHANCED HOMEMAKER DD/SLS/CES

Homemaker assists and supports with various light housekeeping tasks, meal preparation and companionship activities for clients in their homes. Homemaker is used for each individuals tasks that they may not be able to do for themselves. I.e. folding their laundry or making their bed. Homemaker is solely to be used for the person with a SLS/CES WAIVER not for the use of a cleaning service. *Parents and or guardians may qualify to utilize Family homemaker for their services.
